What to Expect
When we welcome you for your first visit, you will be asked to complete paperwork. Your current medications and medical history will be reviewed, and you will receive an in-depth neurological exam. We will conduct further diagnostic testing, such as an EMG or EEG, if needed.
The provider will explain possible procedures and treatments. Together, we will determine the best treatment plan for you.
You will return for follow-up appointments to monitor your progress and adjust medications as needed.
What to Bring to Your Appointment
- Government-issued Photo ID.
- List of all medications you are currently taking (names and doses). Many patients find it easier to simply bring their medication bottles with them.
- Insurance card(s).
- A parent or legal guardian if the patient is under 18. A parent or legal guardian must be present during the entire visit.
- Records from other health care providers, if applicable.
- Completed forms including New Patient Questionnaire and Registration Form (see below for forms).
